I use the Etymotic ER4 and the Ety HF3 for the iPhone. I've tried the triple flangers (painful for my ears), the foam tips (hassle to insert, short life, get fouled with earwax), the mushrooms (much better but ridiculously fragile), Comply tips (still a bit painful with fairly poor isolation), and some Comply knockoffs that were a bit less painful, better isolation and easier to clean, but still get annoying over time.
With the Shure medium olives in my Etys, I can literally wear them all day. They are very comfortable. Cleaning them is as simple as a wet paper towel or a baby wipe. I have no idea how long these will last, but I'm guessing a good long time.
Everybody's ear canal is different, so what works for me may not work for you. But for me, these tips are a godsend. They make my iem's better.

By Anonymous I've tried almost all of the available tips (yellow foam, rubber, silicone) for my Shure SE110 headphones, and in my opinion, these are the best. They are comfortable (I forget I have them in), they do a great job at filtering ambient noise, and sound quality is superb. I only have two small complaints: 1) after a while, they lose their ability to squish down- you compress them and the spring right back into shape before you can stick them into your ear, and 2) they are pricey- $20 for 10 pieces of foam? However, 5 pairs lasts me over a year, so I guess I can live with it...

By D. Tan I've had Shure E3c's for over three years now and have traveled around the world with them (several times, actually). These replacement inserts are made of soft foam, similar to the material used in foam earplugs, and are much more comfortable than both the original yellow foam tips that came with the earphones and the silicone "triple flange" tips that I also purchased earlier. As a result, I can wear the earphones longer on those long flights without developing the earaches that the other tips gave me. I think the Small size might actually be a better fit for me, but the cost of each 5-pack is high enough that I haven't done so yet. I would rather be able to buy a 2-pack for ~$5, as I never replace the tips anyways. Because of the pricing, I'm only giving this product 4 stars instead of 5, but it's highly recommended for Shure earphone owners who have not tried them yet.

By P. Curtain Wonderful buds, not too expensive. The fit over flanges for typical in-ears makes a huge difference in bass response. Having a little stash of them means I don't have to let them get nasty before swapping them out. :)
